CLINICAL OBSERVATION ON THYROID CARCINOMA

Abstract
1. A critical analysis was made of 55 cases of thyroid carcinoma encountered at St. Mary¢¥s Hospital, the Catholic Medical College, in the period of 5.5 years from January 1962 through 1967.
In the same period 173 cases of various thyroid diseases underwent thyroid surgery.
305 cases of various types of thyroid diseases were experienced at the Thyroid Clinic in a period of 18 months from January 1, 1966 through June 30, 1967. In the same period 35 cases of thyroid carcinoma were newly diagnosed. Accordingly, the incidence of thyroid carcinoma is 31. 8 per cent of the surgical thyroid specimens and 11. 5 per cent of the cases seen at the Thyroid Clinic. It is our opinion that the current policy of medical management of Grave¢¥s disease and careful case selection for thyroid surgery account for the high incidence of carcinoma in our series.
2. Ratio of the male subjects to the female was 1 to 5. 4. The age of the patients ranged from 14 to 68 years, the 3rd decade being most predominant.
3. Pathologically, papillary adenocarcinoma was most predominant (27 cases), followed by mixed type adenocarcinoma (17cases), undifferentiated carcinoma(4) and follicular adenocarcinoma (3).
4. Metastasis was present in 23. 6 per cent of the series, found in the regional lymphnodes, the bone and lung, in order of frequency.
5. Lobectomy and subtotal lobectomy were performed in 18 cases and in 15 cases, respectively, outnumbering more radical surgery. Those with metastasis were given irradiation to the thyroid area either with a cobalt unit or with a conventional X-ray unit. Whether our conservative approach is. superior to more radical therapy requires a long term follow-up observation.
